Patients are now able to fill out their forms prior to arriving to Valley Andrology Fertility Center. New patients are required to fill out a Patient Information form before any service can be provided. If a patient is being referred for the cryopreservation of a semen sample, they must also fill out the Cryopreservation Storage Packet.
Important: All dates and patient signatures on all forms are not PDF editable and must be written/signed in the presence of a Valley Andrology employee. Semen destruction forms require notarization unless signed in the presence of a Valley Andrology employee.
